Onboarding any (SQL) server starts by gaining insights on uptime, contact information, stakeholders, technical details like IP Adresses, backup location,etc.
Realservername, IP, Port and type of machine (Production, Test, UAT, ...), downtime, Country, Remote control and contact information are pretty basic pieces of information and often the most used. Password information should be stored in a safe location (link to a password control file)
Important to know where backup's should be stored or where they can be retrieved. Also, not always the backup locations are open for every account, so which account has access to these locations.
In which timezone and at which times we can perform planned or unplanned maintenance and who do we need to contact on beforehand.
Names, Emailaddresses, Telephone numbers, as much as possible information to contact the right person quickly.
On a single server you can store 32000+ databases. Not all have the same downtime requirement or contact information. You can specify - if needed - more in detail per database.
We have made it easy by providing a downloadable (.xlsx) spreadsheet where you can fill in all the information. You can use the template tab as a reference.

You can place all your servers in one workbook or you can create a workbook per server, however is convenient for you.
